I am an in-network provider with Premera, Aetna, and Kaiser. I can provide a Superbill for other insurance companies. To use your insurance: Required at least 24 hours before your appointment: Prescription with diagnostic code(s) and photo of insurance card sent to hello@venetyoga.com or faxed to 206-7912102. This is a billing requirement for all insurance claims - even if your plan doesn't require referrals. Please confirm massage therapy is covered under your plan. Coverage is not guaranteed - your specific benefits depend on your plan details. Insurance sessions are guided by medical necessity to treat the conditions named in your prescription. For full-body or relaxation work, book a self-pay Therapeutic Massage. Appointments without required documentation by the deadline will need to be rescheduled.
